For My Choice Members, More Input Means Independence

My Choice Family Care is an incredible resource for meeting your long-term health and medical needs while also supporting a social, independent lifestyle. Since 2000, we have been helping seniors and adults with special needs navigate the Medicaid system and personalize their care plans. In other words, we help people like you decide what goals are most important, what you need to achieve them, and who will help you along the way.

You can learn about enrolling by meeting with representatives at your local Aging and Disability Resource Center (ADRC), where functional and financial eligibility for the Family Care Plan will be assessed. If you are functionally and financially eligible for Medicaid, you are eligible for Family Care as a cost-effective alternative to skilled nursing or institutional care facilities. If you qualify, the ADRC then provides information about the managed care organizations (like My Choice Family Care) you can partner with. From this point on, you are in control and can start managing your care, your way.

Team Approach to Care Plans

Once enrolled, you will receive a welcome call from My Choice Family Care, and an initial meeting will be scheduled within 10 days. This meeting will be your chance to get to know your care manager and nurse, and they will get to know you—from your background and your future plans, to your medical history and likes and dislikes. You will be encouraged to ask questions and tell your team what is important to you. Then, you’ll work together to develop a plan for adding additional team members, such as family, friends, and local providers, and outline the services that make sense for you. You will also receive a member handbook and valuable information regarding My Choice Family Care, but help is always as close as your care manager and you can request changes as needed.

Since 2000, My Choice Family Care has provided Family Care program options to over 29,400 people in Wisconsin. We currently serve over 8,700 members and have the experience and staff to help you be as active and independent as possible. We know that the right care makes all the difference, and believe that you have a right to choose how, when, and who delivers it.
