My Choice Family Care is a well-established, well-respected managed care organization that administers the Family Care program to seniors and adults with special needs in nine Wisconsin counties. We directly employ over 250 professionals and contract over 1,800 community-based providers to serve our members. My Choice is continually expanding its service area and offerings. If you have a passion for serving vulnerable populations and are interested in joining our network of providers, we encourage you to complete a provider inquiry. Becoming a Service Provider
To become a service provider, you must share our mission and provide value to our members. We are always looking for skilled and supportive assistance, and have openings at all levels, in all service areas. From transportation to supportive home care, residential options, employment, and everything in between, My Choice Family Care is always looking to build on its robust network of quality providers. Our members are encouraged to hire people they are familiar with, including friends and family through self-direction. We support the relationships our members already have, and encourage new ones when possible. As a provider, that’s what it’s all about–relationships that you feel good about, and that are good for you.
On January 1, 2020, My Choice Family Care and Care Wisconsin merged and are now one company. While we work through the process of integrating our operations, we are temporarily pausing on new Long Term Care contracts, including adding of new locations, rate reviews and new AFH certifications. We plan to resume contracting in mid-February 2020. Thank you for your patience as we work through this exciting time.
Start by applying and submitting the necessary paperwork—if you have the required experience and background, you will be sent provider forms. Our contracting team is continually evaluating network needs in our service areas and will be in touch to let you know about current opportunities.
Once contracted, your next steps involve working with both our contracting and claims team. We’ll establish specific rates and services based on the Medicaid fee schedule (where applicable), then provide you with detailed training on authorizations, billing, and the My Choice provider portal tool.
(If this is your first time applying with My Choice Family Care, you will need to register as a new user. After that, log in regularly to check openings in your area, as they change often, depending on members’ needs.)
