My Choice Family Care is Committed to Continuity of Care for Members
Welcome to My Choice Family Care, a managed care organization committed to a better kind of healthcare for people who need it most—seniors, adults with disabilities, and the family and friends who support them. We are a team of professional and community-based caregivers working with Wisconsin’s Family Care program to deliver completely custom services that put members in charge of their health and independence.
If you are functionally and financially eligible for Medicaid, you can become a My Choice member and partner with us to manage your Family Care benefits. We believe that if you need ongoing assistance with daily activities, you should be able to get them when you need them, at a place you choose, and from people you trust. Our goal is to help you design a personalized plan that meets your needs in a cost-effective way, and allows you to connect and contribute to your community.
Excellence Is Our Priority
My Choice has been an advocate for independence since 2000, and has helped thousands of members make informed decisions about their quality of life. Now serving 26 counties and growing in Wisconsin, we take pride in changing lives for the better, and upholding these key priorities:
- To serve those in need
- To act as a trusted steward of Medicaid
- To help people experience true independence
- To offer creative alternatives to institutional care
- To respect individual choice
- To embrace the whole person
- To promote physical, mental, and social well-being
